A fissura labiopalatina é o defeito facial mais facilmente encontrado e implica diversos problemas estéticos e funcionais. O uso de modelos de treinamento é uma opção recomendável no treinamento de jovens cirurgiões, principalmente em cirurgias da cavidade oral, em que o acesso cirúrgico e o manejo dos instrumentos são dificultados. O objetivo deste artigo é descrever um modelo reprodutível de treinamento em fechamento de fendas palatinas, desenvolvendo segurança e habilidade técnica nos jovens cirurgiões para realização da palatoplastia. O modelo de treinamento é constituído de cinco etapas, incluindo aulas teóricas, vivência do ato operatório e confecção do modelo de treinamento. O modelo de treinamento proposto neste artigo permite que o cirurgião se familiarize com o trabalho em cavidade pequena e profunda, aprenda a utilizar lentes de aumento e/ou microscópio, e treine incisões, dissecção e sutura em profundidade, podendo ser confeccionado manualmente e com baixo custo. O simulador proposto permite treinamento realístico em palatoplastia, podendo ser usado para o ensino da técnica cirúrgica, sendo facilmente reprodutível.

Cleft lip and palate is the most common facial defect and causes several aesthetic and functional problems. The use of training models is recommended for training new surgeons, especially for surgeries in the oral cavity where surgical access and instrument handling are difficult. This paper describes a replicable training model of cleft palate closure that helps new surgeons acquire the technical skills required to carry out safe palatoplasty surgeries. The training model is composed of 5 steps, including theoretical classes, monitoring of real surgery procedures, and the preparation of the training model. The training model proposed here allows the surgeon to become familiar with surgery in the small and deep oral cavity; to learn how to use magnifying glasses and/or microscopes; and to perform incisions, dissection, and suturing at a depth. The model can be made manually and inexpensively, and is easy to replicate. The proposed simulator provides realistic training in palatoplasty and can be used to teach surgical techniques.

Esclerose tuberosa é uma doença genética rara, com herança autossômica dominante, associada à formação de hamartomas múltiplos em vários órgãos, como cérebro, pele, pulmões, rins, coração e olhos. Os autores deste estudo apresentam um caso de uma paciente do sexo feminino, com 30 anos de idade, portadora de esclerose tuberosa, apresentando múltiplos angiofibromas em face, tratada com equipamento de alta frequência (radiofrequência), e discutem as opções terapêuticas para tratamento de indivíduos portadores de esclerose tuberosa com extenso envolvimento cutâneo

Tuberous sclerosis is a rare genetic disease with autosomal dominant inheritance, associated with multiple hamartomas in several organs, such as the brain, skin, lung, kidney, heart and eyes. The authors of this study report a case of a 30 years old female patient with tuberous sclerosis, presenting multiple angiofibromas on face treated with high frequency equipment (radiofrequency), and discuss the therapeutic options for treatment of individuals with extensive cutaneous involvement in tuberous sclerosis

INTRODUÇÃO: O espasmo hemifacial (EH) é um distúrbio do movimento caracterizado por contrações involuntárias dos músculos inervados pelo nervo facial, a descompressão microvascular (DMV) é considerada tratamento definitivo, com baixas taxas de recidiva. Os casos refratários tornam-se um desafio para o cirurgião plástico. RELATO DO CASO: MSP, sexo feminino, 53 anos, portadora de EH, refratária à DMV, e com queixas de flacidez dos dois terços inferiores da face. Foi submetida à ritidoplastia, neurectomias e miectomias seletivas. Obteve-se ausência completa dos espasmos, manutenção da mímica facial e rejuvenescimento facial. A ritidoplastia cérvico-facial associada a neurectomias e miectomias mostrou-se eficaz no tratamento do EH.

INTRODUCTION: The peripheral facial palsy is a disease that can leave significant aesthetical and functional sequelae in patients. According to medical literature, Bell's palsy (BP) is the most common cause, representing from 50 to 80% of the cases. However, it must be considered as an exclusion diagnosis. A slightly higher prevalence is estimated in women and its incidence is bimodal with peaks in the third and eighth life decades. Most patients recover within weeks, though a worsening degree may be common in the first 48 hours. Treatment must be performed to prevent complications and the medical procedures are not universally accepted. The following have been mentioned as BP risk factors: arterial hypertension, mellitus diabetes, pregnancy, puerperium and infection caused by type I herpes virus. Although there are several BP associated risk factors, regarding face-lifting surgery there is only one reported case in medical literature. CASE REPORT: In the present study, the authors describe the second case of Bell's palsy after face-lifting surgery.

Cecal volvulus is an uncommon cause of acute bowel obstruction in adults. The mechanism is torsion of the enlarged, poorly-fixed or hypermobile cecum. Patients with this condition may display highly variable clinical presentations, ranging from intermittent, self-limiting abdominal discomfort to acute abdominal pain associated with intestinal strangulation and sepsis. The treatment needs to be individualized for each case, but surgical management is required in almost every case. In the presence of gangrene or perforation of the cecum, resection and primary ileocolic anastomosis is recommended. However, in non-complicated cases detorsion and cecopexy are adequate. The authors report one case of cecal volvulus in a 55-year-old women treated with cecopexy that complicated with septic jaundice.

INTRODUÇÃO: Após sua publicação, o uso do retalho miocutâneo transverso do reto-abdominal para reconstrução de mama (TRAM) tem sido o procedimento com tecido autólogo mais popular para reconstrução mamária, porém não livre de complicações da área doadora. MÉTODO: Foram avaliados onze pacientes submetidas à mastectomia e à reconstrução mamária imediata com retalho TRAM e dissecção mínima da área doadora, utilizando-se pontos de adesão e drenagem a vácuo. Os autores avaliaram as complicações pós-operatórias da área doadora. RESULTADOS: Nenhum caso de seroma, hematoma, infecção, abaulamento de abdome ou hérnia foi identificado, houve uma deiscência de ferida operatória e um relato de desconforto. CONCLUSÃO: Concluiu-se que os pontos de adesão e o sistema de drenagem diminuem as complicações da área doadora do retalho TRAM.

BACKGROUND: After the TRAM flap for breast reconstruction have been published, become the most popular autologous breast reconstructive procedure, although not free of donor site complications. METHODS: Eleven patients underwent to immediately postmastectomy breast reconstruction with TRAM flap and minimal dissection on donor site using quilting suture's and closed suction drainage were analyzed. The authors had evaluated prospectively the abdominal complications of the donor site. RESULTS: None of the patients developed seroma, hematoma, wound infection, abdominal bulge or hernia. One patient developed partial skin loss, and one patient developed discomfort. CONCLUSION: The quilting suture's plus closed suction drainage decrease the complications outcomes at TRAM donor site.
